Indications

Amlohope 5/5 Capsules (Hard Gelatin) is used for: Essential hypertension in patients: whose blood pressure is not adequately controlled on amlodipine or ramipril OR already controlled with ramipril and amlodipine given concurrently at the same dose

Adult Dose

By mouth, 1 tablet once daily

Child Dose

Renal Dose

Renal impairment: CrCl 10-60 mL/min and haemodialysis: Maximum of 5 mg (Ramipril) once daily

Administration

For Oral use

Contra Indications

Hypersensitivity to amlodipine, ramipril, any of the excipients, dihydropyridine derivatives, or angiotensin converting enzyme (ACE) inhibitors Pregnancy History of angioedema associated with previous ACE inhibitor Hereditary or idiopathic angioedema. Concurrent use with aliskiren in patients with diabetes or moderate to severe renal impairment (GFR less than 60 mL/min) Significant bilateral renal artery stenosis or renal artery stenosis in a single functioning kidney Severe hypotension Shock Haemodynamically unstable state Obstruction of the outflow-tract of the left ventricle; Mitral valve stenosis Breastfeeding is not recommended

Pregnancy-Lactation

Interactions

Dantrolene: Concurrent use is contraindicated. Increases risk of cardiovascular collapse. Angiotensin receptor blockers; Aliskiren: Concurrent use is contraindicated in patients with diabetic nephropathy and is not recommended in other patients. May increase risk of hyperkalaemia, hypotension, and syncope. Sacubitril/valsartan: Concurrent use is contraindicated. Increases risk of angioedema. Simvastatin: Significantly increases simvastatin concentrations. When used concurrently, maximum simvastatin dose should be 20mg. mTOR inhibitors, such as sirolimus and everolimus: Concurrent use increases risk of angioedema. Tissue Plasminogen Activators: Concurrent use increases risk of angioedema. Co-trimoxazole (trimethoprim/sulfamethoxazole): Concurrent use increases risk for hyperkalaemia. Lithium: Increases lithium concentrations and may cause lithium toxicity. Concurrent use is not recommended. Drugs that can cause hyperkalaemia, such as potassium supplements and potassium-sparing diuretics: Increases risk of hyperkalaemia. Allopurinol, immunosuppressants, corticosteroids, procainamide, cytostatics, and other substances that may change the blood cell count: Increases risk of haematological reactions, such as agranulocytosis and neutropenia. NSAIDs: Increases risk of renal impairment. NSAIDs decrease efficacy of this medication. Use with caution. Strong or moderate CYP3A4 inhibitors, such as protease inhibitors, azole antifungals, clarithromycin, and erythromycin: May increase amlodipine concentrations. CYP3A4 inducers: May decrease amlodipine concentrations
